Please notify us at least 24 hours in advance if you need to reschedule so that we may accommodate another patient waiting for an appointment. There is a charge for missed appointments and same day cancellations.
Please call your pharmacy for prescription refills. The pharmacy will send the renewal requests to our office. Refill requests received by us before 3:00PM will be processed the same day. Requests received received by us after 3:00PM will be refilled by the end of the next business day. Prescriptions are not refilled on weekends or holidays; please anticipate your medication refill needs.
We refill medications on our patients that are seen on a regular basis. Please make sure that you call to schedule an annual visit if we have not seen you in the past 12 months.

New patients should provide current insurance information at their initial visit.
At every visit, it is important for all patients to inform our office staff of their current insurance coverage. Because patients' insurance coverages frequently change, we will ask for insurance information each time you visit our office. This is necessary for us to submit insurance claims and to make sure they are processed properly.
We participate with most major insurance plans. The following is a list of some of these major plans:
- Aetna US Healthcare
- Anthem Blue Cross / Blue Shield
- United Healthcare
- Banker's Life
- Beech Street Corporation
- Golden Rule
- Medicaid (Connecticut)
- The Husky Program
In order to see one of our Cardiologists, many insurance plans require a referral from the primary care physician. It is the patient's responsibility to have the referral completed prior to a visit. If a referral has not been approved by your insurance plan, you may have to reschedule the visit or become responsible for the charges associated with your visit. Please check with your primary care physician before your visit to make sure they have obtained the required referral from your insurance plan.
Many insurance plans require a co-payment/co-insurance for each visit. Co-payments are expected at the time of service. Many plans do not require a co-payment for testing, however, some plans may require a co-insurance.
Patients are responsible for full payment of the amount billed to them on their monthly statement. If there is a question, please contact our Billing Office, at (203)265-9831.
